All classes other than the Mini SXS Class must turn in a completed W9 at registration.

It is required that Minors have the Minor Waiver be completed, signed, notarized and the original turned over to SXS Sports, LLC at the registration table prior to each event. Failure to have an original document at each event will prevent the minor from participating, no exceptions.

A PARENT OR LEGAL GUARDIAN MUST CHECK-IN WITH MINOR DRIVER AT ALL SXS SPORTS, LLC. EVENTS.

THE 2020 POWER OF ATTORNEY FORM is required to be completed and notarized if the parent or legal guardian is not attending the event with the minor.

Copies will NOT be accepted at the registration table. ONLY the NOTARIZED ORIGINAL will be accepted. The completed form must be received by SXS Sports, LLC BEFORE THE MINOR WILL BE ALLOWED TO PARTICIPATE IN THE EVENT. No registration application will be considered complete until the ORIGINAL notarized form is on file with SXS Sports, LLC. Faxed/Scanned/Emailed applications WILL NOT be accepted.
